       1.  Letter from candidate indicating his desire to be ordained by the EFCA.

       2.  Letter of recommendation from the church of which candidate is a member.

       3.  Letter of recommendation from the church where the candidate is serving.

__     4. Written ordination thesis.

      5. Minutes of ordination council.

       6.  Recommendation from district board or ordination council.

       7.  Current EFCA Minister’s Profile.

       8. Affidavit of completion of required EFCA reading or completion of TEDS History and Polity Course.

       9. Transfer of Ordination Affirmation Questionnaire.
